For hospitals with more than 200 annual births To enroll in PeriData.NetŪ, complete the commitment form and send to WAPC at McConnell Hall, 1010 Mound St., Madison, WI 53715 with a check for $7,500. Click here to view the PeriData.NetŪ Commitment Form. For hospitals with fewer than 200 annual births Through a grant from the University of Wisconsin Medical School Community-Academic Partnership Fund, WAPC is able to extend PeriData.NetŪ to hospitals with fewer than 200 annual births. Active recruitment of the eligible hospitals began in 2005 and will end in 2007.
